Please review and sign this on behalf of yourself (if you are over 18) and/or any children in your care who are using the facilities. You must either be the person listed on the booking or the parent/legal guardian/responsible adults of the person(s) using the facilities.
Where you are not the parent/guardian of that person using the facilities you certify that the parent(s) of that person have granted permission for you to assume responsibility for that person, (their ‘Responsible Adult’) and for you to have the right to sign this waiver and release of liability agreement on their behalf in order to sign the waiver.
I wish to participate in the activities including, but not limited to Inflatable Park access, Inflatables, (collectively hereinafter called ‘the Activities’) organised by The Imagination Street Limited (‘the Company’). I agree with the following:
I am the Parent/Guardian of the child/ren listed below who is/are under 18 years of age. I wish that/those child/ren to participate in the activities including, but not limited to Inflatable Park access, Inflatables, (collectively hereinafter called ‘the Activities’) organised by The Imagination Street Limited (‘the Company’). I agree with the following:
I understand that the Activities are physically demanding and involve jumping and other strenuous actions sometimes involving height, speed, and unpredictable surfaces.
I understand that Imagination Street Ltd does not assess the abilities of individual participants or the suitability of the activities for use by individual participants. I further understand that all participants, parents and guardians must make their own assessment of whether the activities are suitable for use by individual participants and individual participants ability to participate prior to use.
I understand that the activities are used by all age groups at the same time and are not restricted.
I understand that the Activities are a high-impact, full body activity which requires intense focus, awareness of my body’s strengths and limitations, awareness of the environment around me, and extreme caution always.
I acknowledge that the Activities can be dangerous and there is a risk of personal injury when undertaking such Activities and participation can result in serious injury or death.
I/he/she/they use the Activities entirely at my/their own risk.
I acknowledge that participation in the Activities can result in personal injury including, but not limited to, friction burns, cuts, abrasions, bruising, muscle strains, twists, sprains, dislocations, broken bones and paralysis and accept that these injuries can occur through general use of the equipment and not always or necessarily as a result of defective equipment or negligence of the Company.
I acknowledge and accept that the Activities require a moderate level of fitness and can be physically demanding and require physical effort and I/he/she/ they should not undertake the Activities unless I/he/she/they am(is/are) physically able to.
I understand that I/he/she/they am/is/are responsible for my/his/her/their own actions and /or involvement in using the activities.
I understand that I/he/she/they is/are not permitted to hold/cradle babies and/or small children whilst using any of the activities, as this may result in severe injury to both parties.
I understand that Imagination Street Ltd does not provide direct supervision of participants when using the activities. I understand that Imagination Street Ltd does not provide training or tuition to participants in the use of the Activities.
I/he/she/they will not attempt flips or other extreme moves whilst using the activities.
In the unlikely event of an accident, or loss or damage to any personal effects, I acknowledge that the Company will not be liable for any direct or indirect loss, damage or injury arising from or in connection with the Activities (except for death or personal injury caused by the Company’s negligence), or with my, our failure to comply with the safety instructions and/or directions of Imagination Street Ltd, and I waive all and any claims against the Company in this respect.
I acknowledge and accept that CCTV images of all park entrants will be monitored and recorded throughout the premises for the purposes of public safety and crime prevention.
By entering Imagination Street Ltd and participating in ‘The Activities’, I hereby consent to:
I acknowledge that I (and my child/ren in my care if applicable) have been provided with safety rules and advice of the Company in relation to the Activities and that I have read and fully understand the above and accept the terms of participation freely and voluntarily without any inducement prior to signing/submitting this form.
I have signed a risk acknowledgement & waiver form as a participant in the Activities and I agree that entering into this agreement indicates permanent waiver and release of liability. I understand that this agreement will be kept on file for future visits. I understand that if I wish to revoke this agreement I must contact Imagination Street and obtain their agreement to such revocation in writing. I understand that this waiver, release of liability and image release is intended to be as broad and inclusive as permitted by the laws of England and Wales and agree that if any part/s of this agreement are held invalid the remainder will continue in full force and effect.
If in any doubt as to your ability to participate safely in ‘The Activities’ please seek advice from your GP.
Specific Medical Information
Customers with Down’s Syndrome prior to taking part in such activities, will be asked to obtain evidence that a GP or Paediatrician has screened them using the screening test developed by the British Gymnastics Association for Atlantoaxial Instability. Approval for participation that is signed by the GP or Paediatrician must be provided before taking part in any activities.
Customers with Achondroplasia (disproportionate Dwarfism) should not participate in bouncing. They should not perform gymnastic skills which involve jumping or rebounding.
Customers with Epilepsy
Flashing lights are used in our sessions that might affect some people with epilepsy. Customers with uncontrolled epilepsy will be asked to obtain their GP’s approval before bouncing considering the following:
Bouncing should only take place when the individual is accompanied by a person ages 18 years or over, who must have knowledge of the condition and be able to recognise an attack.
Bouncing should NOT take place if the person with uncontrolled epilepsy feels unwell.
Feet first when going down slides, please make sure the end of the slide is clear, only one person to go down the slide at a time.
DO NOT bounce on the hatched marked area at the front of the arena, this is for entry and exit only.
Socks must be worn at ALL TIMES.
No Climbing on the walls between activities or around the arena.
No Rough play, wrestling, tackling or shoving.
No Sitting or Lying on the inflatable.
No Food, drink or chewing gum allowed on the inflatable.
No bouncing under the influence of drugs or alcohol.
Remove all items from pockets, including keys before entering the inflatable.
No jeans, belt buckles, studs on clothing or jewellery.
DO NOT leave clothing lying on the inflatable.
DO NOT participate if you are pregnant or are suffering from any injuries.
All injuries must be reported to Imagination Street staff before leaving the building.
At the end of the session, all participants must get their wristband hole punched by a member of staff before leaving the inflatable.
In the event the inflatable starts to deflate, please exit the inflatable quickly and calmly using the nearest exit and inform a member of staff. Only re-enter the inflatable once told by a member of staff that it is safe to do so.
